放射性栓塞治疗神经内分泌肿瘤肝转移的临床应用进展

神经内分泌肿瘤(NET)确诊时21-50%患者已存在远处转移,其中肝脏是最常见的转移部位。神经内分泌肿瘤肝转移(NELM)的存在是影响患者生存预后的最重要因素。NELM确诊时,60-70%患者呈现双叶多灶性病变,不适合手术切除。由于全身治疗手段有限,肝脏靶向治疗存在临床需求。经动脉栓塞、化学栓塞和放射性栓塞等血管介入治疗在NELM治疗中应用日益增多。近年来,放射性栓塞(也称选择性内放射治疗,SIRT)因其具有良好的肿瘤缩小效果、毒性反应有限和科学证据不断增加而受到广泛关注。本综述阐述了放射性栓塞的基本技术原理,总结了NELM放射性栓塞治疗的现有文献证据,并讨论了在NELM治疗中应用放射性栓塞的注意事项、挑战和新进展。
